By signing this permission slip/waiver with your electronic signature, you acknowledge that your child will actively participate in Pamper Me Princess LLC activities.Your child will be receiving spa services including but not limited to mini manicure, mini pedicure spa services, the use of sea salt soak, polishing, scrubs, scented oils/lotions, whirlpool foot baths and etc. Your child may get a mini spa facial with the use of cleanser, exfoliate, masque, cucumbers, and/or moisturizer. Your child may also have a makeup application with the use of eye shadow, eyeliner, mascara, blush and/or lip gloss or lipstick. There will be pictures taken/video recordings during the party. Photos will be uploaded to an online gallery and/or social media accounts for promotional purposes. Pamper Me Princess LLC cannot be held responsible for how photos are shared. You will further grant permission to use without recompense, any photographs, videotapes, audio recordings, or any other recordings for promotion of Pamper Me Princess LLC. During the event, your child may be going up and down stairs to get in and out of the bus and may maneuver back and forth on the bus interior floor. Pamper Me Princess LLC is not responsible for any damages to personal items, accidents or injuries that may incur while under our supervision. Pamper Me Princess LLC cannot be held responsible for any allergic reactions caused by food consumption or interaction. You assume all risks and hazards incidental to participating in the spa/party activities and waive, release, absolve, and indemnify and agree to protect, defend, and hold harmless Pamper Me Princess LLC, its owner, independent contractors, and employees for any claim arising out of any injury/medical injury to your child to the fullest extent.
